Assuming that you meet the eligibility of FMLA (more than 12 months of service, more than 1250 hours worked in the past 12 months), you could be entitled to a FMLA.
To qualify for a continuous Family and Medical Leave of Absence, you would need to submit paperwork (typically a Certification of Healthcare Provider for Employee’s Serious Health Condition) that states you have/had a serious medical condition that lasted at least 3 consecutive CALENDAR days (not work days) that required continuing treatment. “Continuing Treatment” is defined as a doctor’s office visit/ER visit that requires at least one additional treatment within 30 days.. i.e. another office visit, prescription.
It sounds to me that an Intermittent Family and Medical Leave may be more appropriate. Your physician would need to complete the same paperwork notifying your employer that you suffer from a serious medical condition that MAY make you unable to perform the essential functions of your position. For example, your physician could say that due to your illness, you could have flair-ups that may require you to be absence upto 3 hours a day, 2 times a week.
