Gout is a disease caused by congenital disorder in uric acid metabolism. Uric acid is a product of waste material found in the urine and blood from the chemical processes of the body. In case of gout, the monosodium urate or more popularly called the uric acid crystals are being deposited on the articular cartilage of joints, tendons and surrounding tissues. The crystal deposits are due to the increase of uric acid concentration in the blood stream.

Gout is characterized by inflammation of the tissues that could also burst through the skin. That is why people suffering with gout sometimes have sinuses discharging a material white as chalk. Moreover, gout is also known as metabolic arthritis.

Gout Signs and Symptoms

Gout has visible symptoms. Persons with gout usually suffer with swelling and pain in the joints especially in the hands a nd feet. There is also an The presence of agonizing, sudden, unexpected burning pain, redness of skin in the areas affected, warmness and stiffness of the joints are also felt. Sometimes, patients also become feverish. The crystals inside the joints and the inflammation of the tissues around the joints cause the pain. Where Gout Attacks?

Gout usually attacks in the upper and lower extremities especially when these parts of the body lack exercise. The lack of exercise could cause the crystals to be easily deposited because the joints are at rest and the blood is also flowing into a smoother flow. However, almost 75 percent of first attacks of gout are recorded in the big toe. But aside from not, gout is not limited to the big toe, it could also attack other joints in the feet and hands like in the ankle, heel, instep, knee, wrist, elbow, fingers, and spine. Gout in the spine often results in backaches and difficulty in moving because the spinal column becomes stiff. Poor blood circulation in fingers due to earlier injury could also lead to gout.

Since the kidney is responsible in filtering the blood stream, a malfunction of the kidneys could also lead to gout because of the high levels of uric acid that the system is not able to eradicate.

Men between the ages of 40 and 50 are most prone in developing gout. Protein-rich foods are often the cause of high levels of uric acid. Alcohol intake could also affect the level of uric acid and can cause acute attacks of gout. Moreover, the development of gout could also be a result of hereditary factors.

Gout commonly presents with a sudden onset of redness, swelling and excruciating pain of the great toe joint during the night. Gout is a painful joint condition caused by hyperuricemia (high levels of uric acid in the blood) resulting in the precipitation of needle-like crystals in the joints. The prevalence of gout has been steadily increasing over the past two decades is now estimated to be 1-2% of the U.S. population. Gout is more common in men, but in recent years, the incidence of gout has doubled in women (1,2). The common causes of gout are diet, overproduction of uric acid and renal disease.  Common risk factors include family history, male gender, a high protein diet, heavy alcohol consumption, high blood pressure, kidney disease, metabolic syndrome, diuretic use and obesity. Hyperuricemia has been suspected to increase the risk of heart disease by increasing inflammatory factors that damage blood vessels (3).

Treatment varies for acute and chronic gout. For acute gout flares, the standard treatment is a course of anti-inflammatory medications, steroid injections and/or joint aspiration. Allopurinol is commonly used for treatment of chronic gout and prevention of acute gout attacks. Febuxostat is a new medication recently approved by the FDA. In long term studies over a 5-year period, febuxostat was shown to reduce uric acid levels and substantially decrease or eliminate acute gout attacks (4). A study last year in the Archives of Internal Medicine showed 500 mg of Vitamin C per day reduced the incidence of gout by 15% (2). Researchers at the Arthritis Research Centre of Canada in Vancouver found long-term coffee consumption is associated with a lower incidence of gout. Individuals who drank four + cups of coffee per day had the least risk of developing gout (5). The authors did not recommend starting to drink coffee for those individuals who do not already drink coffee.

The relationship between heart disease and gout has been suspected to be more coincidental than causative, until recently. Recent research suggests uric acid may be a causative factor in heart disease. Uric acid activates the immune response and the inflammatory process. This reaction can be seen in an acute gout flare involving a painful, red, hot, swollen great toe joint. Uric acid is both an antioxidant and a pro-oxidant. This means that although uric acid may quench free radicals and reduce oxidative cell damage, it also increases mediators which cause damage to blood vessels and increase the risk of heart disease (6).  

The most effective prevention of gout is the modification of diet and lifestyle. Specific foods and beverages, including red meat and alcohol, can trigger an acute gout attack. Obesity increases the risk of gout and other conditions which increase the risk of gout, such as hypertension and metabolic syndrome. Weight reduction and daily exercise are both appropriate preventative treatments for gout. Other recommendations include limiting intake of sugary beverages and foods with added sugar, consuming whole grains, nuts, legumes, fruits, vegetables, moderate amounts of lean white meats and low fat dairy and oily fish (7). You may want to talk to your doctor about adding a vitamin C supplement or increasing your coffee intake.

Gout is one of the most common forms of arthritis (joint inflammation). Gout is condition that results from crystals of uric acid depositing in tissues of the body. Gout usually affects the first metatarsal phalangeal joint of the big toe (hallux) or the ankle joints. Gout most often affects the big toe but can also affect the ankle, knee, foot, hand, wrist and elbow. Approximately one million people in the United States suffer from attacks of gout. Gout is nine times more common in men than in women. It predominantly attacks males after puberty, with a peak age of 75. In women, gout attacks usually occur after menopause.

It occurs more frequently in countries that have a high standard of living. The most common sign of gout is a nighttime attack of swelling, tenderness, redness, and sharp pain in your big toe. There may also be the feeling that the skin around the area is stretching or tearing. There seem to be a number of factors that can cause people to develop gout. Some medications can prevent uric acid from leaving the body. Cyclosporine which is a medication used to suppress the body’s immune system and prevent rejection after organ transplant, also increases the risk for developing gout. This can cause a build up of uric acid crystals.

Alcohol and certain foods can trigger a gout attack. Events such as strokes, heart attacks or surgery may also cause gout. Gout is strongly associated with obesity, hypertension, hyperlipidemia and diabetes. Because of genetic factors, gout tends to run in some families. A variety of treatments can help you manage your gout. Traditionally treatment for acute gout has consisted of colchicine , which can be effective if given early in the attack. Gout often is treated with nonsteroidal anti-inflammatories (NSAIDs) such as naproxen (e.g., Anaprox, Naprosyn) and indomethacin (e.g., Indocin), usually for 3–7 days.

To prevent future attacks, your doctor can prescribe a medicine to reduce uric acid buildup in your blood. Changing the way you eat can help you conduct your gout. Eat a healthy mix of foods so you get the nutrients you need. Limit foods like meat, fish, and beer. Drink plenty of water and other fluids.Life style changes such as limiting alcohol consumption, and limiting meals with meats and fish rich in purines can also be helpful in controlling gout. Weight loss may be very important this condition. You should also avoid certain animal proteins, such as: seafood; liver; kidney; heart; gizzard; sweetbreads; meat extracts; and gravy.

Herbal remedies for gout have become very popular in recent years. There is very little evidence right now that shows alternative therapies are effective for gout. Uricinex is a breakthrough herbal product that we have recently discovered. Blueberries are high in anti-inflammatory compounds called anthocyanins, which help ease the pain of gout. Other herbal treatments that have shown some success include vitamin C – Taking supplemental amounts of vitamin C helps some people to reduce the uric acid in their blood. Taking 2-3 teaspoons of black cherry juice a day helps some people. Cherries are a popular home remedy for gout.
